Please label all of your child's accessories and belongings (sweatshirts, hats, towels, water bottles, lunch boxes etc.) with their first and last name. We have a very large lost and found box that grows every day!
All lost & found items are donated at the end of the summer.
The Parks & Recreation Department and Q-Club staff are not responsible for any lost or stolen items. This includes toys, trading cards, jewelry, electronic devices (i.e. cell phones, mp3 players, e-readers, game systems etc.) and costumes and props associated with theme days or other activities. WATER/SQUIRT GUNS ARE PROHIBITED. Again, please label all of your child's accessories and belongings with their first and last name!
There are 2 snack/drink breaks each day, one at 10:00 a.m. and a second at 11:00 a.m.
NO NUT PRODUCTS PLEASE!! Some children have very serious allergies to these products
We encourage good hydration among the children and staff. It is a good idea to send your child with a water bottle that can be refilled throughout the day.
Each disciplinary problem will be handled on an individual basis. Rules and regulations are necessary, but will be kept to a minimum and equally enforced for everyone. Q-Club utilizes verbal reminders, time-outs and formal written reports to enforce the rules of the program. Discipline reports and "Think About It" Sheets are designed to formally document a child's behavior that has been repeatedly disruptive and/or has put themselves or someone else (another child or counselor's) safety in jeopardy. Once a discipline report or a "Think About It" sheet, has been completed, the child's parents will be contacted to discuss the report and inform them of the specific behavioral problem. Please note the steps the Q-Club staff take, regarding discipline, below:
- Discipline Report #1: Parents contacted and informed of behavior, as well as how it was handled and whether it may impact future participation.
- Discipline Report #2: Parent contacted and child will not be allowed to participate for a minimum of 2 days and a maximum of 5 days. Parent and child must sign a Behavior Contract before returning to the program.
- Discipline Report #3: Parents are contacted and child will be disallowed from further participation for the year. A refund will not be issued.
Note: If the severity of the behavior warrants, a child will be disallowed from participating in the program INDEFINITELY, regardless of the number of prior discipline reports completed. The safety of the program participants is of the utmost importance!
*Per the discretion of the Q-Club Director and the Director of Parks & Recreation, if a child is removed from the Q-Club program for the duration of the summer, this individual may not be permitted to register for the program in the future.
Hold Harmless Agreement
By registering for Q-Club, and paying the program fee, you agree to indemnify, save harmless and waive, the Town of Queensbury, the Town Board, the Department of Parks & Recreation, employees and volunteers thereof, for any responsibility should any accident or injury occur to your child as the result of participation in the Q-Club program or while using school or recreation program facilities.
Q-Club staff will NOT store or dispense medication of any kind.
Epi-Pens & Inhalers
With a Patient Specific Order (PSO) on file, children deemed mature enough in age and ability may self-administer required medications in the case of an emergency. Children will only be allowed to bring emergency inhalers and emergency Epi-Pens in original containers (with the patient's name, prescription fill date, precautions, storage requirements, expiration date, directions for use, pharmacy and physician names and numbers) to the program if a Patient Specific Order is received at the time of registration. This is a written prescription or note from your child's physician. If an emergency occurs and the child must self-administer, a counselor/ health director designee will complete the incident form. If emergency medication is used it will be properly logged.
Children will be allowed and encouraged to re-apply sunscreen/insect repellent to themselves throughout the Q-Club day, if parent permission is granted (on the emergency card) at the time of registration. Parents are responsible for providing the sunscreen or insect repellent that they would like their child to use. Sunscreen must be FDA approved, over-the-counter and will not be used for medical treatment or to treat illness.
Each child participating in Q-Club must have all registration materials approved prior to starting camp. These materials include emergency medical and contact information, as well as a copy of each child's current immunization records. All children must be fully vaccinated to New York State Department of Health standards in order to attend Q-Club. Immunization records are required at the time of registration. All immunization records will be kept on file for the duration of the summer.
All Q-Club staff are certified in American Red Cross CPR, First Aid, and AED. In case of an injury, staff are trained to respond accordingly. All minor camper injuries (small cuts, bumps and bruises) shall be recorded in the Q-Club Medical Log. Any accident or injury requiring more than a just a band-aid or an ice pack, will be documented on a Q-Club Incident Report. Staff will contact the parent/guardian within 24 hours, if an incident report is completed.
Any camper suspected of having a communicable disease shall be suitably isolated. Parents will be notified of all injuries and illnesses which warrant a formal report. Note: Q-Club staff are NOT permitted to administer medication of any kind, including but not limited to: topical ointments, antibiotic ointments, inhalers, epinephrine pens, and insulin.
Children must be "checked in" and "checked out" of Q-Club each day.
Authorized Pick-Up Individuals
Any individual attempting to pick up a child from Q-Club MUST be on the child's authorized pick up list. The authorized pick-up list will be completed by a parent when registering the child for Q-Club. Any additions or deletions to the list made throughout the summer must be made by logging into your e-PACT account and updating authorized pick up permissions. STAFF MEMBERS WILL NOT, UNDER ANY CIRCUMSTANCES, RELEASE A CHILD TO AN INDIVIDUAL THAT IS NOT ON THE CHILD'S AUTHORIZED PICK-UP LIST. FURTHERMORE, STAFF MEMBERS WILL NOT RELEASE A CHILD TO AN INDIVIDUAL WHOM DOES NOT PROVIDE PHOTO IDENTIFICATION AT THE TIME OF PICK-UP. Please see specifics below.
When children are picked up at the program location, the adult picking them up must come up to the check in/check out table in order for the child to be safely released to the individual. Staff members will ask whoever is picking up the child for a photo ID to ensure that the individual is verified "okay" by the parent for pick up. To make this process easier, there will be two stations set up for check in/check out. One for children with the last name beginning with A-L, and another with children whose last name begins with M-Z. A child will not be allowed to leave the activity location unless accompanied by a parent, guardian or other authorized individual and properly checked out.
Walking Or Riding Bikes Home
A parent/guardian must provide permission on the child's e-PACT registration form, if the child will be walking or riding their bike home from Q-Club. Children that are allowed to walk or ride their bike home will be released at 12:15pm each day, unless otherwise approved. The child must sign themselves out with staff, at the check-out table, before they will be released.
All children riding the bus to or from Q-Club must be registered for the Q-Club program and registered for the bus. A child will NOT be permitted to ride the Q-Club bus until he/she is formally registered.
Bus Stop Locations
Please be aware and understand that there are certain limitations to the Q-Club bus service. Q-Club buses cannot accept requests to stop at personal or private street addresses. This is due to time restrictions. The buses will only pick-up at designated street corners and regional stopping points. Please refer to the bus schedule to determine the closest bus stop location to your home.
A parent/guardian must be present at the bus stop when a child is being dropped off after Q-Club, unless permission to be dropped off without a parent/guardian present is indicated on the child's e-PACT registration form. If there is not a parent/guardian present at the bus stop, and a child's form indicates that they cannot walk from the stop, the child will remain on the bus and be taken back to the 4/5 school cafeteria, at the duration of the bus run. The child will need to be picked up by a parent/guardian in the 4/5 cafeteria.
Bus Safety While Riding
Per the NYS Department of Health, all children are required to wear their seatbelt at all times while riding on the bus. Q-Club staff will ride with the children and will supervise them once they board and are checked-in. Children must follow all Q-Club rules as well as the rules of the Q-Club bus driver at all times while riding. Failure to follow these rules will result in termination of bus privileges WITHOUT A REFUND.
Children that wish to participate in trips or events must be registered for Q-Club and have paid the program fee before attending a trip or event. Cost for each trip includes transportation, supervision and admission.
Field Trip Registration
Field trip registration begins on the first Monday of the Program at 10:00 a.m. Field trip registration will take place at the Queensbury Parks & Recreation Department located in the Town Office Building (742 Bay Road, Queensbury) and also online. Please refer to each trip summary to determine whether the trip is age appropriate for your child. Some trips may not be suitable for all children (i.e. long bus rides, water trips, age limitations etc.) Also, please pay special attention to the trip necessities (lunch/drink, pick-up and drop-off times, locations etc.) of each trip.
Field Trip Registration Deadlines
Trip registration deadlines are the close of business on the day prior to the scheduled trip (unless otherwise noted). REGISTRATION WILL NOT BE ACCEPTED THE DAY OF THE SCHEDULED TRIP. PAYMENT: ONLY CASH OR CHECKS ARE ACCEPTED (made payable to the Town of Queensbury).
Field Trip Transportation
Children who participate in Q-Club field trips or events must ride to and from the scheduled activity on the Q-Club bus. With the exception of the Gurney Lane Cookout and Night Watch trips, children are not allowed to be dropped off or picked up from the location of the field trip.
Field Trip T-Shirt Color: It is our goal to provide a safe environment for all children when visiting a field trip destination. Having a NEON YELLOW designated t-shirt makes a difference for the staff to better identify all Q-Club children participating in the trips. Your cooperation is appreciated! Neon yellow Q-Club T-Shirts are available for purchase ($8 each) at the Q-Club store. We accept cash or checks made payable to the Town of Queensbury. The Q-Club store is located in the Arts & Crafts Room (4/5 Cafeteria) during Q-Club hours.
Field Trip Refund Policy
Refunds will only be issued if the cancellation is done prior to the trip. A $5.00 cancellation fee, per trip, per child will be assessed to all refund requests made. In the event that the department cancels a trip for any reason, or when there is no rain date scheduled, a full refund will be issued.
